Application for Change of Visa Type – Based on Family Association or Accompaniment

Important Note:
Permission may be granted for a period not exceeding 90 days, and the applicant must have at least 15 days remaining on their current permission to stay. Applications submitted after this period will not be accepted.

Required Documents

1. Application Form
   
   

   • Form TM.86 – For foreigners currently holding a Tourist Visa or Transit Visa
     
     

   • Form TM.87 – For individuals entering Thailand under Phor.15, Phor.30, or Phor.90, without a visa
     
     

2. Copy of Passport
   
   

   • Including the personal information page, visa stamp, entry stamp, extension of stay stamp (if applicable), and TM.6 card
     
     

3. Photograph
   
   

   • One recent 4 x 6 cm or 2-inch front-facing photo (no hat, taken within the past 6 months)
     
     

4. Application Fee
   
   

   • 2,000 Baht
     
     

5. Copy of Passport and Work Permit
   
   

   • Belonging to the foreign national whom the applicant intends to reside with
     
     

6. Certificate of Juristic Person Registration
   
   

   • Including company objectives
     
     

7. Value-Added Tax (VAT) Registration Certificate
   
   

8. Shareholder List
   
   

   • Showing the current shareholders of the juristic person
     
     

9. Proof of Family Relationship
   
   

   • One of the following:
     
     

     • Marriage certificate, birth certificate, or documentation proving father, mother, or child relationship
       
       

     • Must be certified by the embassy of the applicant’s home country in Thailand and the Legalization Division of the Ministry of Foreign Affairs (Tel: 02-575-1056 to 9)
       
       

     • Alternatively, an official certification letter from the applicant’s embassy or consulate stating that the applicant is a family member or part of the household
       
       

     • Certification may also be provided by the Department of Protocol, Ministry of Foreign Affairs
       
       

10. Personal Income Tax Filing (Por. Ngor. Dor. 1)
    
    

    • For Thai employees of the sponsoring company, for the past 3 months
      
      

    • Must include a complete list of all Thai employees
      
      

11. Latest Personal Income Tax Filing (Por. Ngor. Dor. 91)
    
    

    • For the foreign employee whom the applicant is residing with
      
      

Submission Guidelines

• Applicant’s Documents:
  All submitted documents must be signed and certified on every page by the applicant.
  
  

• Company or Partnership Documents:
  All corporate documents must be signed and certified on each page by an authorized director as listed in the company registration, and stamped with the company seal.
  
  

• Submission of Originals:
  To ensure smooth and efficient processing, applicants are kindly requested to submit complete sets of documents along with all original documents to the immigration officer at the time of application.
